“One conversation I have almost every week in my clinic is with couples who wish they had come six months earlier.” – Dr Rajeev Agarwal
In over two decades of fertility practice, I have sat across hundreds of couples in my clinic; hopeful couples, exhausted couples, couples who have already been through one failed cycle and have no clue why. In almost every conversation, I find myself talking to couples about decisions that were made before the treatment even began.
IVF is the final step in a long process, and the way you prepare for it matters more than most people realize. The mistakes I’m going to share with you are not made out of carelessness but anxiety and misinformation.
Fertility is a process that begins long before embryo transfer, injections, or even your first consultation. Oocyte quality reflects months of follicular development. Sperm health reflects the quality of your health over the preceding 72 to 90 days. In fact, implantation itself is influenced by the environment we create in the body before conception.
This is the philosophy behind the PREPARE method that I developed. It is a structured framework for preconception counseling in Kolkata or anywhere else that works through planning, review, evaluation, prevention, assessment, reinforcement, and enhancement. Every couple who comes to me benefits from this framework because it asks the right questions at the right time and tells you about how to prepare for IVF treatment successfully.
“Waiting for problems to appear allows age-related decline, metabolic changes, and preventable risks that compound quietly. In fact, by the time a couple seeks help, some of that valuable time cannot be recovered.”
Here are some common mistakes before IVF that I’ve noticed through my practice.
Most couples wait far longer than they should. A woman under 35 with no known fertility concerns should seek evaluation after 12 months of trying. If she’s over 35, that window is six months. If there is a known condition like PCOS and endometriosis, regular cycles, or more, waiting for even six months can mean losing precious time. An early conversation with a fertility specialist in Kolkata could give you the right information that might change your entire approach.
Male factor infertility accounts for almost 40 to 50% of all infertility cases. Yet, when couples come to me, it is almost always the woman who has undergone testing and the man who has not. Fertility is a joint biological process. A semen analysis is non-invasive, quick, and can give critical information about sperm count, motility, and morphology. If male factor is contributing to difficulty in conception, we need to know about that on day one and not six months into the treatment cycle. It is an important step if you are wondering about how to prepare for IVF.
Infertility is one of the most emotionally demanding experiences that a couple can go through. I understand that, but I also see how unmanaged stress can become a physiological problem that can disrupt cortisol levels, affect sleep, and create hormonal imbalances. All this can directly impact fertility outcomes. That is why I always ask couples about their emotional state. It is not because I doubt their strength, but because managing stress is as much a part of IVF preparation as any medication protocol. Therefore, counselling, mindfulness, and honest conversations matter much more than couples realize.
I do not say this to dismiss the internet, but fertility forums, social media reels, and algorithm-driven health content are not reliable medical advice. I’ve seen couples refuse science-backed protocols because something that they read online contradicted it. Please use the internet to ask better questions. Do not use it to replace your doctor’s clinical judgment.
I understand that IVF treatment is a significant financial expense. The concern is valid and very real, but cost alone is a dangerous filter. The factors that actually determine outcomes are lab quality, embryology team, experience, protocols that your doctor designs for you and the aftercare. It is important to choose your infertility clinic based on clinical outcomes, transparency, and the depth of the consultation you receive, and not just the pricing.
Couples often hesitate to ask questions as they are worried about taking too much time or feel that the questions are too personal, but a good fertility specialist wants you to ask. Your questions are going to help me understand what you know, what you are afraid of, and where the gaps lie. I suggest it would be a good idea to come to a consultation with a written list. You can ask about IVF success rates at the clinic, the number of embryos you’re likely to get, and what happens if the four cycles do not work. You can also inquire about egg freezing, sperm freezing or even a surrogacy centre in Kolkata as backup options. The more informed you are, the better decisions you will make throughout the process.
Before I order a single medication or test, I sit down with every couple and tell them one thing: IVF is not a guarantee. Its effectiveness depends enormously on how well you prepare for IVF. Your biology sets the starting point, and your preparation is going to decide how far we can take it. I also tell them that the couples who do best are not the ones with the easiest cases. They are the ones who come prepared, ask questions, take care of themselves, follow a good lifestyle, and stay honest with their doctor.
Here is a pre IVF checklist for couples trying to conceive.

IVF success is not purely a matter of biology. The mistakes I see most often are preventable, and preventing them begins with an honest conversation with your specialist, who will take the time and make an effort to understand your full picture.
If you are considering IVF or want to understand your fertility better, I encourage you to begin that conversation now and not when the problem appears. Successful conception often begins long before pregnancy. Don’t just conceive, prepare to conceive.
Physical preparation can include addressing modifiable risk factors like healthy weight, stopping smoking and alcohol, beginning relevant supplements, and completing diagnostic investigations. Emotional preparation is also important, therefore you should seek counselling if needed.
The most impactful changes are stopping smoking (both partners), eliminating alcohol, reducing high caffeine intake, achieving a healthy BMI, improving sleep quality, and managing chronic stress. These changes begin 90 days before a planned cycle as this time period reflects the time it takes for egg and sperm maturation to complete a full cycle.
Age is one of the most significant factors, particularly for women’s ovarian reserve and egg quality. Beyond age, sperm quality, uterine health, quality of the embryology lab, lifestyle and the stimulation protocol used all play important roles.
Always look for transparency in reporting clinical outcomes by age group. Assess the quality of the consultation and check if the laboratory is equipped with advanced embryology capabilities or not. Also see if the clinic offers a range of services including IUI treatment, egg freezing, surrogacy, and comprehensive infertility treatment. Cost is an important factor but it should never be your primary filter.
